A case management nurse ensures proper coordination of medical services for patients by overseeing the performance of all health care professionals. A case management nurse, when used in connection with managed care, administrates all the activities of evaluating the patient, planning treatment, making referrals, and planning follow-up care. This ensures patient care is continuous and comprehensive and helps to coordinate payment schedules.
Case management nursing is practiced by nurses who have specialized formal, post-secondary education and who function in highly autonomous and specialized roles.
